QuietCare (sm) is an exclusive, patented, state-of-the-art home based security system which can report changes in activities of daily living (ADL) that could signal a medical emergency. The system consists of five small, wireless motion sensors placed in strategic locations throughout the home, such as the bedroom, the bathroom and the area where medications are stored.

The system reports information to TSI for round-the-clock analysis. The system first learns the senior’s normal ADL pattern, so that it can recognize activities that are out of the ordinary. If certain changes are significant, you can be alerted via phone, text message, e-mail or pager.

Plus, you can review your client’s or family member's ADL anytime on a secure Web page. This personalized site includes reports on ADL, such as kitchen activity, bathroom activity, mobility and activity where medications are stored. Household temperatures are monitored as well. It can allow caregivers to intervene early, helping to avoid medical emergencies and hospitalizations.

QuietCare Can Help Monitor These ADL’s:

• Did the senior begin moving around the house in the a.m.?
• Did they exit the bathroom okay?
• Has their nighttime bathroom routine changed?
• Has their overall activity level decreased significantly?
• Are they visiting the area where they keep their medications?
• Is the household temperature too hot or too cold?

Common Conditions that are monitored by QuietCare include: Dementia, Diabetes, Fall Prevention, Sleep Deprivation, and Urinary Tract Infections.

Assisted Living Facilities and Other Patient Care Organizations Desiring a Self-Monitored QuietCare Program directly from Living Independently Group (LI).

The LI self-monitored QuietCare Systems means that alerts are sent directly to the nursing station, security station, or to the attendant’s pager or cell phone. The NPI QuietCare Provider may offer both the Self-Monitored QuietCare Program internally and offer the Monitored Program as an out-reach service to extend its presence into the community.

NPI-AUTHORIZED PROVIDER AGREEMENT

There is no obligation attached to the signing of the NPI Acceptance Form. However, signing the Form does provide you with access to all incentives, now and in the future. Sign, and fax toll-free to (866) 234-3269. Tax exempt organizations should include a re-sale or non-profit (501c3) certificate with their Acceptance Form.

Upon Activation the following will occur:

• You will be contacted by a TSI representative. Your Agency does not have to purchase any system unless you need one for demo or re-sale purposes. Your cost is detailed in the Incentive Structure information above.
• Schedule on-site training - QuietCare only. On-site training and complete web-based training is provided free.
• Your Agency generates a revenue share for each QuietCare installation and/or for each GA911 sale based on the what you want to charge the Subscriber - See Incentive Structure. You simply enter the installation fee and monthly monitoring charge in the Subscriber Agreement. Your Agency will be paid directly by TSI.
• Living Independently's new distributor TSI does the monitoring, billing, and collections. There is no cost to you. Monitoring stations are located in both Illinois and Florida to nationally monitor installed QC Systems. GA911 contacts, automatically, the local 911 System.
• A limited number of marketing brochures will be provided free. Your Agency can also download and customize QC and GA911 brochures for your needs. NPI will market your organization through Eldercare-net.org.
• There is no capital investment.

TWO QUIETCARE ALTERNATIVE PROGRAMS FOR NPI AFFILIATES THAT WILL GENERATE A MARKET PRESENCE AND/OR REVENUE

NPI PROVIDER INSTALLER

TSI markets and sells QuietCare nationally. If TSI sells a QC System your Agency will be contacted by TSI. Install the QC System and receive $84.95 for the installation from TSI. Your Agency can retain the client for Agency specific services.

NPI REFFERAL ORGANIZATION

Provider signs no forms and receives no incentives. NPI has been informed by many NPI Community Partners that they do not want to received any revenue share, they just want the capacity to offer a choice to their clients and to generate a market presence. This alternative direct pricing falls under Eldercare-Net referrals.

In short, If a TSI distributor or an NPI QuietCare Provider is located in your service area, NPI will have the QuietCare systems installed on your behalf.
